Statutory Declarations and Affidavits
A Statutory Declaration is a legal document which the maker must sign in the presence of an authorised witness, declaring that the information it contains is true and correct.

A standard form Statutory Declaration is available to download as either a PDF or Word file. Please complete the form but do not sign it. It must be signed in the presence of a person qualified to witness Statutory Declarations:
When presenting at the University Solicitors Office to make a Statutory Declaration or Affidavit, please ensure you bring photo identification with you.
The University Solicitors Office offers a document certification service to staff and students.
Please ensure you have made copies for certification and that you have original documents with you when attending.
