Important Forms

Once the student has finalised the internship details with a suitable host organisation, they are required to complete an online application.

This is submitted to the Experiential Learning Officer for approval and the student will be advised of the enrolment procedure.

During the first day of the internship, the student should meet with their host supervisor to complete the Duties Outline (27 kB)*. This document formally sets out the duties and/or tasks the student can expect to undertake during the internship. It also indicates the level of supervision and guidance provided by the host supervisor.

The Duties Outline should be submitted to the Experiential Learning Officer no later than one week after the commencement of the internship.

At the end of the internship the host supervisor should complete the Host Evaluation (36 kB)*. This document allows for feedback and comment on the student's accomplishments during the internship.

The student will also complete a Student Evaluation (31 kB)*, which allows for a reflection of their internship experience. This evaluation is important in terms of the Faculty's continuous improvement practices.

Both evaluations are returned to the Experiential Learning Officer at the end of the internship.
