Statutory Declarations and Affidavits

A Statutory Declaration is a legal document which the maker must sign in the presence of an authorised witness,
declaring that the information it contains is true and correct.

A list of persons authorised to witness Statutory Declarations can be found on the Victorian Department of Justice website.  Details of University staff (26KB) qualified and willing to witness Statutory Declarations and receive Affidavits is available to download.

A standard form Statutory Declaration is available to download as either a PDF (42KB) or Word (27 KB) file. Please complete the form but do not sign it.  It must be signed in the presence of a person qualified to witness Statutory Declarations.
