Additional Documentation

During your application through the Course and Scholarship Applicant Portal you will be requested to provide additional documentation.  Below are the requirements for your course.

Not supplying these documents at the point of application may result in a delay in the consideration of your application.

Providing supporting documents

You can upload documentation at anytime in the application process.  You can also log back in and submit documentation after submission of your application.

You can find details on who can certify your documents through Certified photocopies for supporting documentation.

Course Specific Additional Documentation

Education Off campus applications

Complete and submit the Unit Selection Sheet (28 KB) (pdf) to indicate which units you would like to be enrolled in.

Nursing and Midwifery applications

Postgraduate nursing specialty courses must complete and submit the appropriate hospital preference form below:

H773 Master of Nursing Practice (Nurse Practitioner) course applicants must complete and submit the:

All applicants should obtain a copy of the Honours booklet and internal expression of interest form by emailing - sci-tech@deakin.edu.au

Credit for Prior Learning

If you wish to apply for credit for previous studies as part of your online application, please complete the Credit for Prior Learning application form (88 KB) and submit this with your application by scanning and uploading the form. Alternatively you can submit it using the Supporting Documents Cover Sheet (140 KB).
