Application process

Eligibility:

If you are currently a registered Division 1 nurse/midwife (including Division 3 and 4) you are eligible to apply.

Guidelines:

We welcome your application to the 2011 Leadership in Nursing and Midwifery Awards presented by Deakin University and Health Super.

Step one: download a nomination/application form (Doc 182 KB)

Step two: complete your application

Your application MUST:

1. include all personal details
2. address all application questions
3. be submitted as a word document (PC or Mac)
4. be provided in english language
5. include a copy of your photo
