Application to review my library record or to ask for a reduction or waiver of invoice, or overdue points.

An application form may be submitted if you would like to notify the Library that:

  • an item that is still listed on your library record has been returned
  • an item listed on your library record was never received

You may also request a reduction or waiver of a Library invoice, or for overdue points you may have received.  Reasons for your request need to be submitted with the application and you may be asked to provide supporting documentation.

If you have not submitted an application to review previously and your request is due to extenuating circumstances then your application may be accepted and processed quickly. Other applications may take longer to review but you will be contacted by a library staff member within 2 to 3 working days.

Your application may not be accepted for a number of reasons, which may include:

  • you have failed to update your current email or mailing address and so did not receive any library notices.
  • you have lent the item to someone and he/she returned it late or failed to return the item.
  • you have returned a 2 hour loan item to another location other than that from which it was borrowed from.

You will be able to continue to borrow while your application is being processed.
