Opinio licence request form

The request for an Opinio licence must be approved by a senior manager within your area (e.g. Divisional Director, General Manager, Dean, HOS).  It is expected that there will be a survey ‘subject matter expert’ (SME) who has undergone Opinio training within the work area requesting a license.

All staff and students involved with the administration of surveys should be familiar with the University’s Survey Evaluation Framework and in particular the surveys operational policy and Surveys procedure.
