Human ethics forms and guidelines

All new applications for projects submitted to the Human Research Ethics Committee MUST include at least the following four documents:
  • The National Ethics Application Form (NEAF)
  • The DU-HREC Victorian Privacy Supplement
  • The DU-HREC Capacity to Consent Supplement
  • The DU-HREC Plain Language Statement and Consent Form (using the template below)
Submissions may also require an ionising radiation supplement.

National Ethics Application Form (NEAF)

NEAF is an online, web-based ethics application form. Please go to http://www.neaf.gov.au complete your application.

Please be aware that students are not to be listed as the Principal Researchers on NEAF.

Supervisor(s) must be listed as Principal Researchers and students must be listed as Associate Researchers.

Please note that you will need to register to access NEAF on-line (click here to go directly to the registration page). This will involve creating a user name and password. Detailed information on how to use NEAF is provided on-line once you have registered.

NEAF Important Notice
From late August 2008 Version 2 of the NEAF will become available for use. From that date, all new NEAF applications must be created in Version 2. Current Version 1 applications cannot be imported to Version 2 and must be completed by 1 December 2008 at which time Version 1 will be locked and remaining applications will be inaccessible. It is recommended that researchers save copies of applications created in Version 1 for future reference. NEW
