Work Safety Assessment
Complete a Work Safety Assessment prior to commencing research work involving chemical, biological, animal, radiation, process (plant) or field work hazards.

For Exempt Dealings approved from 2004 onwards, submit a progress report by 5pm on Friday 26 July 2013.
Note: a new application is required for Exempt Dealings approved before 2004.

Use this form to request LBC approval for new personnel or changes to approved facilities.
For all other variations/modifications you will need to submit a new application (NLRDs cannot be varied or amended once approved).
This annual report is required for all approved NLRDs and covers 31 October 2011 -31 December 2012. 2012 Annual reports are due Friday 15 February 2013.
