Important: if you change/discontinue your course during the trimester please email with details.

All note taking for Classes will be completed online, and will not be completed in person on-campus.

For Seminars/Pracs, please advise whether your attendance will be online or on-campus.

By submitting this request form, you are agreeing to the Student Responsibilities outlined in the Information for Students Working with an Academic Support Worker (ASW). The DRC will review the ASW services allocated to you if you do not fulfill the student responsibilities.

Deakin University is collecting your personal information on this form for the primary purpose of identifying appropriate adjustments to support your studies. It will also use de-identified information to enable Deakin to continue to improve our support services.

You are not required to provide the information requested, however if the information is not provided, Deakin may not be able to complete your request for support.
