Manual Special Consideration application form

This form is for use where a previously granted exam for Special Consideration cannot be attended as a result of exceptional circumstances.

All applications for special consideration should be made via StudentConnect. However, if you don’t have access to StudentConnect (e.g. if you have intermitted or don’t have a current enrolment), please proceed with completing the form below.

Please note the following before making an application:

  1. Applications must be submitted within 3 University working days of the Exam date.
  2. Appropriate current supporting documentation showing exceptional circumstances is required.

Please note that Special Consideration can only be granted for the same task once and where extraordinary circumstances (such as hospitalisation) prevent you from attempting an exam.

Granting of Special Consideration does not automatically entitle a passing grade for any assessment or unit.

Please provide details of the situation that lead to this application. Limit responses to 200 words or less. Supporting documentation must also be provided below and this may include a more detailed response.

Supporting documentation must be in English. Documentation provided in other languages will not be accepted. Please upload PDF (preferred) or Word (DOC, DOCX) files no more than 10MB in size. Documents should be combined where possible into a single file, including any images.

Applications on medical grounds are to be presented on the Deakin University Medical Certificate where possible.

A Statutory Declaration may accompany your application but is not sufficient in itself and therefore must be accompanied by further verifiable support documentation.

Applications without supporting documentation attached will not be considered.
