Application forms


Offer acceptance forms

If you are a student from an Assessment Level 3 and 4 country, you must also attach a completed Country assessment levels 3 and 4 checklist, when you accept your offer, along with the Offer Acceptance and Payment of Fees form, and provide requested evidence so that Deakin can issue you an electronic Confirmation of Enrolment (eCoE).

Authorised Deakin representatives need to also complete and attach an Application checklist for representatives, along with the usual student applications forms, for all applicants to any on-campus award and packaged courses.

If your students are from an Assessment Level 3 country, you must also attach a completed Country assessment levels 3 checklist when you accept your offer, along with the offer acceptance and payment of fees form, and provide requested evidence so that Deakin can issue you an electronic Confirmation of Enrolment (CoE).
