Vaccination and blood test checklist for Health Care Workers (HCW) 2013

Please use the checklist below to guide you in fulfilling vaccination and blood testing requirements for Deakin University related clinical placements. Print off and tick as you complete each requirement.

5 x Tetanus and diphtheria or tetanus / diphtheria/ pertussis vaccination records  
1 x Recent pertussis vaccination record (Boostrix / Adacel or Boostrix / IPV / Adacel / Polio)  
3 x Minimum Polio vaccination records (plus one within the last 10 years)  
2 x Measles, mumps and rubella vaccination records OR blood test results indicating protective immunity  
2-3 x Hepatitis B vaccination records and / or positive blood (Hbsab) test result  
2 x Hepatitis A vaccination records OR positive blood test result  
1 x Mantoux testing certificate or documentation (within the last 12 months)  
2 x Chicken pox vaccination records or positive blood test result  
1 x Meningococcal C vaccination record  
1 x Influenza vaccination record (within the last 12 months)  

Please note:

  • Try and obtain all previous vaccination and / or blood test records (including baby / childhood and / or vaccinations completed at a doctor's surgery, local council, school or hospital) prior to your first appointment.
  • If you cannot obtain all necessary vaccination/blood test records please discuss this with a relevant staff member in the School of Nursing, Medicine or Dietetics.
  • In Australia records of vaccinations completed at school are generally available from the local council of where you went to school.
  • International students should make every attempt to obtain copies of past vaccination/blood test records from their family, doctor, clinic, hospital or vaccination organisation.
  • Please bring all vaccination and blood test records to each appointment.
  • Please bring your valid/current Medicare or Overseas Student Health Cover (OSHC) card to each appointment.
