COVID-19 vaccination

This Privacy Statement explains how your personal and health information related to your COVID-19 vaccination status will be collected, used, disclosed, stored and disposed of.

Vaccine information

Deakin University is collecting your personal and health information confirming your vaccination status (Vaccination information) for the primary purposes of managing the risk of COVID-19 transmission on Deakin campuses as they reopen to staff and students. Collection of your Vaccination information is in line with the Covid-19 Vaccination Procedure. You are not required to provide the Vaccination information requested, however, if you do not it will affect your ability to come on to Deakin campuses for work or study.

Manner of collection

Online submission

When you sign into the Vaccination information webpage via Deakin’s secure single sign on, your sign-on will link to DeakinPeople in the case of staff; or Callista in the case of students. Your first and last name and email address will be autofilled into the Vaccination information form via this linkage.

When you upload the document that contains proof of your vaccination status (vaccination document), a copy of the vaccination document will be retained until sighted by an authorised staff member. Once sighted, your vaccination status will be recorded in DeakinPeople or Callista, as applicable, and your vaccination document will be securely deleted.

In person at Student Central – students only

If you present your certificate to an authorised staff member in person, that staff member will open the Vaccination Information form, autofilled from Callista.

Once sighted, your vaccination status will be recorded in Callista. Your Vaccination information will be accessed by a limited number of authorised staff members who have a need for access relating to health and safety on campus.

Use and disclosure

Deakin will use your Vaccination information for the following purposes:

- to validate and record your vaccination status

- to plan campus reactivation, including allocation of teaching and learning spaces

- to communicate with you about your Vaccination information

- to monitor compliance with Deakin’s vaccination requirements

- to prepare aggregate information on the vaccination status of Deakin staff and students.

Your Vaccination information will be accessed by a limited number of authorised staff members who have a need for access relating to health and safety on campus, including validating your vaccination document.

Deakin will disclose your Vaccination information where required by law including, when required, to authorised officers acting under a direction of the Minister for Health.

Security and disposal

Deakin has technical and organisational measures in place to protect the personal and health information it holds from unauthorised access, use, modification and deletion.

Your vaccination document will be held in Deakin’s enterprise systems until it has been validated, after which it will be securely deleted. Your Vaccination information will be held as long as it is required for the purposes of collection, after which time it will be securely destroyed in accordance with the University’s Retention and Disposal Authority.
