Leave of absence application form

Please note:

• You can only be approved for a leave of absence during teaching periods (weeks 1–11 of the trimester/semester).

• A leave of absence is not required during the study period or the end-of-unit assessment and exam period. However, if you plan to travel offshore or return to your home country, you must have completed all assessments and have no scheduled exams or outstanding end-of-unit assessments.

• If you are a commencing student (in your first study period), you are required to be onshore and have commenced studies before any short-term leave of absence can be considered.

Terms and conditions *

• I understand that a short-term leave of absence is for a maximum of ten (10) working days. If granted, it will not qualify me for special consideration or assignment extension. I will contact my units chairs to advise them of my absence. I have read the conditions of a short-term leave.
