Deakin CAPs recognises the time constraints when seeking supporting documentation for University processes such as Special Consideration or academic progress. At peak times, it can be difficult to find a timely appointment with a Deakin Counsellor for such purposes.
If you have not engaged with a Deakin Counsellor during the past 4 months, it is unlikely that you will be provided with supporting documentation from our service. In these cases please consider the following options before making an appointment with a Deakin Counsellor.
Option 1
Get the supporting documentation from your current psychologist / mental health practitioner or General Practitioner / Doctor who is familiar with your situation.
A letter from the psychologist / mental health practitioner must be on their letterhead.
If you can't get a letter from them in time then please try option 2 or 3.
Option 2
Make an appointment with a General Practitioner / Doctor:
Should you require a list of local bulk billing GPs, then call the Deakin Health and Wellbeing Centre on 92445577.
Option 3
If you don't have time to obtain any of the above, you can submit a Statutory Declaration.
Part of this process involves you completing are written statement that is signed in the presence of an authorised witness.
The authorised witness must cross reference your signature with some form of photo ID. They must print or stamp his or her name, address, and title under section 107A of the Evidence Act 1958 (Vic).
Examples of authorised witnesses: Pharmacist, Justice of the Peace, Police Officer, Legal Practitioner, Medical Practitioner, Dentist, Court Registrar, Bank Manager.
