Pre-placement documents

Before you commence placement, you will need to have a valid Working With Children Check and completed Pre-Placement Occupational Risk Checklist. Find instructions about how to complete these forms here (PDF). Please refer to the Commencing Student Guide for further details. All consent forms must be completed by 27 March 2026.

The Working with Children Check and completed Pre-Placement Occupational Risk Checklist form must be completed by 15 May 2026.

The following are required later, and you will be contacted at that time: Flu vaccination, Hand hygiene certificate, Fit2work police check, First Aid and CPR certificate and Interstate Working with Children Check.

Below you'll find the 10 compliance forms to be completed. Please work your way through the list. Documents must be uploaded to InPlace (Clinical Placement System) by 27 March 2026 (except for WWCC and Pre-Placement Occupational risk checklist).
