• understand this is a voluntary and unpaid role

• agree to follow Deakin’s exam supervision instructions when supervising the exam/s
• am at least 18 years old
• am of sound mind and have the physical capacity to monitor the exam
• will not be under the influence of drugs or alcohol during the exam
• am able to competently read and write in English and understand the exam instructions
• am not an immediate family member of the student
• am not a direct employee of the above student
• am not enrolled as a current student of Deakin University
• agree to treat all student information I receive confidentially and not disclose information unless approved to do so as part of the supervision process. This is in accordance with the Privacy and Data Protection Act 2014 (Vic) and the Health Records Act 2001.
